About The Ponytail Lift
The Ponytail Lift is not just one single surgery; rather, there are different procedural levels and variations within the Ponytail classification. Each Ponytail procedure addresses different facial regions and corrects varying degrees of aging. However, each procedure uses endoscopes, which provide clear visualization to more effectively reposition the tissue vertically along the natural bone structure. The goal of each Ponytail procedure is to achieve the skin tightness of a high ponytail. (2)
Ponytail Lift I
The Ponytail Lift I addresses earlier signs of aging and minimal skin excess in the upper-face/midface. This lift procedure is ideal for younger patients who are seeking subtle lifting, contouring, and beautification.

Preparation
After establishing your Ponytail Lift plan, Dr. Fairchild will outline your preparation responsibilities. These will be specific to your treatment plan and medical history. In general, she recommends that patients:
- Pause the use of blood-thinning medications and supplements
- Cease smoking or nicotine use
- Limit alcohol intake in the weeks prior, and avoid it entirely a few days before treatment
- Arrange transportation home and assistance during the initial recovery period
- Take time away from your career or other responsibilities to recover
- Avoid harsh chemical exfoliants or new aggressive treatments
- Minimize sun exposure
- Pick up any prescriptions you may need during your recovery
By abiding by these guidelines and any others you receive, you put your body in a healthy, strong state for surgery, which makes for a smooth procedure and optimal results.
Procedure
When you first arrive for your procedure, Dr. Fairchild will have you sit comfortably and naturally in an upright position. She will carefully mark incision points within the hairline, ensuring she has optimal access while keeping them hidden. After this, team members will place you under twilight sedation (local anesthesia with IV sedation).
Once you are asleep and comfortable, Dr. Fairchild will create three to five incisions in the scalp near the temples. Next, she will insert an endoscope through one of the incisions. With this, she receives high-definition visuals of the underlying facial structures on a monitor, enhancing precision as she can visualize facial structures in real time. Once she identifies the deep tissues and ligaments on the monitor, she will use a tiny and precise cutting tool to safely release and untether the retaining ligaments. Instead of excising the tissues completely, she will elevate and reposition them to a higher and more youthful position. Each Ponytail procedure differs in its specific focus and execution after this point:
- If Dr. Fairchild is performing the Ponytail Lift I, she will mainly address minimal aging in the temples, upper cheeks, and outer brows. She will pull and tighten tissues in a single upward and backward vector without skin removal.
- If she is performing the Ponytail Lift II, she addresses early jowling or mild neck laxity by utilizing deeper and broader internal release patterns through the hairline incisions.
- During the Ponytail Facelift I, Dr. Fairchild addresses upper and midface aging with incisions hidden within the temporal and scalp hairline. Through these incisions, she lifts the forehead, rotates the brows, and raises the cheeks using a deep plane technique without any lower face work.
- The Ponytail Facelift II will target early jowling or mild submental fullness and mild skin laxity. Dr. Fairchild will create scalp incisions and additional 2-cm endoscopic access incisions behind the ears for a deeper lift. This variation will employ deep-plane dissection techniques to raise tissues in the upper/midface in addition to reducing jowls.
Once Dr. Fairchild repositions the tissues, she will close the incisions using fine sutures that minimize scarring. After applying the appropriate dressings, she and her team will monitor you until you can return home.
Fat Grafting and Lasers
The Ponytail Lifts/Facelifts can include fat grafting, depending on whether you require volume restoration. Fat grafting uses liposuction to remove fat from a donor site elsewhere on the body (creating a dual benefit of reducing that site). Once obtained, Dr. Fairchild reinjects it into the face. This technique adds long-lasting, beautiful fullness to the face.
Your Ponytail procedure can also include laser treatments to smooth, tighten, and refine the skin. Laser energy will stimulate collagen and elastin production, which will help enhance healing and tighten the skin further. Additionally, lasers can help minimize the formation of scars.
Recovery
Before going home to recover, Dr. Fairchild will provide you with an information packet that provides step-by-step post-procedure recovery instructions. After your procedure, you will experience mild to moderate bruising and swelling. During this initial recovery period, it is important that you prioritize rest and elevate your head and shoulders. Swelling and bruising typically reach their peak during the third and fourth days, and you may experience numbness around the scalp and face. Because of this numbness, you must avoid using hair dryers, as you can risk burning your skin. You will also need to minimize your activity for the first few weeks. Dr. Fairchild’s recovery information packet will outline everything you need to know about rest, medications, elevation, and more.
Dr. Fairchild will usually schedule a follow-up appointment after five days to remove initial dressings and check your incisions. By the second week of your recovery, the majority of your bruising will clear, and swelling will reduce significantly. At this point, you may also return to work and engage in light daily activities. However, you must still avoid any strenuous activity, heavy lifting, and driving. The overall tightness and bruising will begin to fade once you reach the fourth week. Your tissues will continue to settle into their final positions, and contours will become more refined over the following two to three months. The scars behind the hairline will continue to fade, eventually blending seamlessly into your hair.
Results
Once residual swelling dissipates, you will be able to enjoy a beautiful, refreshed, and lifted appearance. Your cheeks will appear elevated, and drooping skin in the midface will now be tighter, revitalized, and more youthful.
